Plan Options
Select Advantage Discount Plan
The Avesis Vision Discount plan provides vision discounts rather than insured benefits, making it worry-free, hassle-free and paper-free. There are no claim forms for the Participant to file, no waiting periods, prior authorizations or insurance provisions.
Select Exam Plus
Provides covered employees and eligible dependents with periodic professional vision
examinations and materials. The examination is a complete analysis of the visual
functions, including the prescription of corrective lenses where indicated. The
covered eye exam is insured and the hardware is discounted at participating providers.
Select Advantage Plan
(Fully Insured) (Scheduled Allowance) Provides for an examination, standard lenses and frames
from a high quality selection that is covered in full after the applicable copay up to the plan
maximums. Or the employee or dependent can choose to receive a generous allowance towards the
purchase of contacts in lieu of frames and lenses for out of network providers. This is the
most comprehensive vision plan of the four.
Select Advantage Materials Only Plan
(Scheduled Allowance) Provides standard lenses and frames from a high quality selection that is covered in full after the applicable copay up to the plan maximums at participating providers.
Or the employee or dependent can choose to receive a generous allowance towards the
purchase of contacts in lieu of frames and lenses for out of network providers. This
plan works well for a group that already has an eye exam under the group's medical plan.

Plan Options
Coast to Coast Discount Vision Plan
The Coast to Coast (CTC) Vision Plan has contracted with over 12,000 eye care locations
nationwide. Members save on eyeglasses, contacts, eye exams
and surgical procedures. The CTC provider panel is the most comprehensive in the U.S. and
includes ophthalmologists, optometrists, independent optical centers and national chain
locations such as participating LensCrafters, Pearle Vision, Sears Optical, Target Optical,
and JCPenney stores.
20/20 Insured Eye Exam + Coast to Coast Discount Hardware Plan
This plan will pay up to a maximum benefit for the eye exam. Members can use any vision
center or doctor, the eye exam benefits remain the same in or out of the network. For
hardware benefits, members can use any of the 12,000 CTC providers to receive discounts.
20/20 Select Vision Plan
(Insured) (Indemnity) This plan will pay up to a maximum benefit for the eye exam and hardware. Members can
use any vision center or doctor, the eye exam benefits and hardware benefits remain the
same in or out of the network. If the member used the participating providers, they
will receive the Coast to Coast Discount first, then the copays and insured benefits
are applied. If there is a balance, the member pays the balance out of pocket.
20/20 Select Materials Only Vision Plan
(Indemnity) This plan will pay up to a maximum benefit for the hardware. Members can use any
vision center or doctor, the hardware benefits remain the same in or out of the network.
If the member used the participating providers, they will receive the Coast to Coast
Discount first, then the copays and insured benefits are applied. If there is a balance,
the member pays the balance out of pocket. This plan works well when the eye exam benefit
is covered under the group's medical plan.
